TITLE: Pharmacist
DESCRIPTION: Participates in the day-to-day pharmacy operation activities as a licensed Pharmacist.
Reports To: Director of Pharmacy
PRIMARY FUNCTIONS:
1. Performs in the pharmacy in the following: prepare medicines, dispense prescription medicines, check dosage, ensure that medicines are correctly and safely supplied and labeled, and maintains a register of controlled drugs for legal and stock control purposes. Provides pharmacy services for Health Center, Home Infusion, Ambulatory Infusion, and Hospice programs.
2. Counsel and advise the public on the treatment of minor ailments and any adverse side-effects of medicines or potential interactions with other medicines/treatments
3. Ensure the ordering and safe storage of medical products and, in some cases, arranging the delivery of prescription medicines to patients.
4. Responsible for the management of pharmacy staff (technician) during service hours.
5. Represent the agency before professional and community groups to promote services and participate in networking activities.
6. Educate, communicate and coordinate with physicians, dentists, nurses, management and other staff regarding orders and issues related to patient care and pharmacy related topics.
7. Evaluate equipment, supplies, and medication being used for pharmacy effectiveness as well as cost effectiveness.
8. Follow established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer (Vice President of Patient Services) immediately. Failure to comply with HIPPA policies and procedures will result in disciplinary action, up to and including termination.
9. Other duties as assigned by supervisor.
QUALIFICATIONS:
- Bachelor’s degree from an accredited pharmacy degree program approved by the Illinois State Board of Pharmacy required.
2. Licensed without restrictions to practice pharmacy in the State of Illinois required.
3. One year of professional experience in the compounding, dispensing, and preservation of drugs and medicines equivalent to a Pharmacist, including prior or concurrent successful completion of the required pharmacy internship.
4. Two years of supervisory experience preferred.
5. Health policies and requirements must be met.
