JOB DESCRIPTION
TITLE:
Transition Specialist ~ West Campus
QUALIFICATIONS:
1. Bachelor Degree from an accredited college or university in rehabilitation counseling,
special education, education, guidance and counseling or a closely related human Services
field. .
2. Minimum of one year's experlence in rehabilitation, special education, or closely related
human services field.
REPORTS TO: Director of Special E:ducation
JOB GOAL:
To manage and coordinate the Secondary Transitional Experience Program
(STEP) and the Transilion Specialist Program.
PERFORMANCE RESPONSIBILliIES:
Interview and gather information on students to determine their eligibility for services, the extent and nature of their disability and their need for the feasibility of their undertaking rehabilitation Services.
Maintain necessary case documents on students required by the Department of Human Services/Office of Rehabilitation Services in order to serve disabled students through the Secondary Transllional Experience Program (STEP).
Maintain a continuous case, finding program through contacts with school personnel; establish screening and referral processes to ensure that all eligible students are offered vocational rehabilitation services.
Work cooperatively with the District's administration, pre-vocational coordinators and other pertinent staff members
to ensure appropriate services to disabled students.
Attend staffing, multi-disciplinary conferences, IEP meetings. and other decision-making meetings that pertain to the Senior STEP student's vocational rehabilitation and transition program.
Approve job sites for STEP students, as necessary.
Provide vocational counseling to STEP students to develop an appropriate post-graduate program for each student.
Provide post-graduate assistance and follow-up services through the
Department of Humanservices/Office of Rehabilitation Services. Coordinate DHS/ORS services with school personnel and community services.
Assist Special Education Director in the development of the yearly contract and budget for the Transition Specialist and STEP grants.
Provide workshops to the District regarding DHS/ORS policies and procedures as necessary.
Communicate with the students' parents/guardians on an as needed basis.
Adhere to all policies and regulations of the special education district, DHS/ORS, and the School Code of Illinois.
Must be able to work in a bilingual Spanish/English environment.
TERMS OF EMPLOYMENT:
12 month - the TS is employed as is any other teaching staff person in the District. However, the TS is required to report for work one week earlier than regular teaching staff and to work one week longer at the end of the school year.
A regular teacher works 180 days. The TS works 190 days plus 20 days during the summer for a total of 210 days. The normal contractual teacher holidays apply including Thanksgiving, Christmas, Easter Break, and the remaining applicable on day national holidays.
RATE OF PAY:
Commensurate with the duties performed and with other professional employees in the District.
EVALUATION:
Performance of this job will be evaluated annually in accordance with the provision of the Board's poicy on evaluation of non-certified employees.
